Transition from Children's to Adult Services

The Children’s Physiotherapy Service provides care for children aged 0-18 years in mainstream schools, or 0-19 years old (or when they leave special school) for those in special educational settings.

The transition process begins at the age of 14 where future plans will begin to be discussed. When the time for transitioning into adult services approaches the physiotherapy team will ensure you are fully informed and supported during this process. Depending on your child’s needs, the Children’s Physiotherapy Team may refer onto the appropriate adult services for ongoing support.

Once transitioned to adult services it will be the responsibility of the main carer of the young adult to seek a referral to the adult orthotic service as required. This referral can be provided via the GP.
