Market Weighton GP Practice Nominated for National Award

Market Weighton GP Practice has been shortlisted for the General Practice Awards 2020 for the identification and care of people with Heart Failure. The nomination in the category ‘Clinical Improvement: Chronic Conditions’ acknowledges how the team have identified and managed of patients with heart failure (HF) in primary care. The practices approach to improving care for those with heart failure has enhanced patient care and improved the patient-clinician relationship in a busy primary care setting.

The General Practice Awards are the most prestigious awards for primary care professionals. Awards will be presented in a glittering online ceremony on 26 March 2021.

Sue Smith, Senior Practice Manager said: “To be a finalist in this category makes us very proud and recognises the excellent work we have done and are continuing to do every day. The Heart Clinic has received great feedback from patients and their families and we look forward to sharing share our approach with colleagues regionally and nationally so it can improve care for people living with heart failure.”
