Mental Health Triage and Assessment Team
The Mental Health Triage and Assessment Team provide routine mental health triage and assessment to people who are living in Hull and East Riding of Yorkshire who are registered with a local GP. The service is for 18 to 64 year olds.
You may be referred to this service by your GP or another professional. You may be contacted to complete a triage, where a clinician will contact you by phone. This call is to establish your current needs and to identify the appropriate care for you, which may be delivered by another organisation which you are signposted to.
If your triage identifies you require a further assessment, you will have an appointment booked for this, which aims to further understand your experiences and plan what care is required in collaboration with you and your family.

Self/carer referral:
If you are a patient or carer wishing to access mental health services, please contact the Mental Health Advice and Support Line (0800 138 0990). Alternatively, you can speak to your GP or other health professional for advice.
GP/primary care referral:
If you are a GP or linked to a primary care general practice, please refer using the electronic referral system (eRS).
Other professionals:
If you are a professional wishing to discuss a referral, please contact the professional advice line where a clinician can further advise on the best course of action. If you wish to make a routine referral and cannot complete this by eRS, find more information on our webpage.
Please note, all referrals for routine triage and assessment require the explicit consent of the person being referred. If this is not obtained, the referral will not be processed.
