Help us build our new Patient Information Library
We've created a new online Patient Information Library to make it easier for patients, carers, families and colleagues to find trusted health information and wider support in one place.
The library will become the central home for patient information while also bringing together links to trusted information from partner organisations across health, local government and the voluntary, community and social enterprise (VCSE) sector.
Many people accessing our services need more than healthcare alone. They may also need support with housing, finances, employment, caring responsibilities, wellbeing or community services. By bringing together trusted information from across organisations, we can make it easier for people to find the help they need, whenever they need it.
The library will contain two distinct types of information, each clearly labelled so users know where it has come from.

Patient information created and maintained by the Trust, ensuring patients, carers and colleagues always have access to the latest approved version.

Partner information and signposting
Trusted information from partner organisations, including local authorities and VCSE organisations.
Your resources will remain on your own website. We will simply link directly to them, meaning users will always access the latest version that you maintain.

Adding your organisation's resources to the library will help us to:
- Improve access to trusted information across our communities.
- Support joined-up care across health, local government and the voluntary sector.
- Help health and care professionals signpost people to the right support more quickly.
- Improve access to services that address the wider determinants of health and help reduce health inequalities.
How to submit a resource
If your organisation provides information that could benefit our patients, carers or staff, we'd love to include it in the library.
Submitting a resource is quick and straightforward using our online submission form.
You'll be asked to provide:
- Your organisation's details.
- The title and a short description of your resource.
- A link to the webpage where the resource is hosted.
- Relevant categories and tags.
- A named contact.
- A review date so we can periodically check that the information remains up to date.
Our team will review all submissions before they are published.

Our Patient Information Library is now live!
We're currently populating the library and are now inviting organisations to submit resources.
We would be grateful if you could also share this opportunity with colleagues or partner organisations who may have resources that would benefit our patients, carers and communities.
By working together, we can create a single, trusted source of information that helps people find the right support at the right time, wherever they are in their health and care journey.
