Transition of GP Services in East Riding of Yorkshire

Humber Teaching NHS Foundation Trust is transitioning GP services at Market Weighton Practice, King Street Medical Centre in Cottingham, and Humber Primary Care in Bridlington to new providers due to changes in the national primary care model. Patients do not need to take any action at this time.
This decision was made in collaboration with representatives from the Integrated Care Board and aligns with their strategy to support integrated neighbourhood care. It reflects a shared commitment to commissioning services that are sustainable, future-focused, and able to meet population health needs over the long term.
The Trust is currently in discussions with potential providers and will notify patients once the new provider is confirmed. All practices will remain at their current locations, and patients will automatically transfer to the new provider.
A formal letter will be sent to all patients explaining the transition, confirming the new provider, and detailing registration options.
The transition is being carefully managed, and a communications strategy will ensure patients and stakeholders remain informed.
