Short Term Support Team

Who do we work with?

Autistic people and people with a learning disability aged 18 and under
who are on the Dynamic Support Register.

What will we do?

Help people and their families/carers to get the right support at the right
time from education, health and social care.

How long will we offer you support?

We will work with families for up to 6 months.

How will we support you? 

We will...

• Offer support in a way that works best for you.
• This could be coming to visit you in person in a space you feel comfortable in, talking over the phone or email or speaking with parents/carers
• Help you to set goals and support you to plan how to achieve them
• Put you at the centre of any planning
• Listen to what is important to you and make sure your voice and wishes are heard
• Support communication with other services
• Ask how you would like us to support you
