Humber and North Yorkshire Dynamic Support Keyworker Service

Who can have a Humber and North Yorkshire Dynamic Support Keyworker?

Our Dynamic Support Keyworkers work with:
• Autistic children and young people.
• Children and young people with a learning disability.

To have a Dynamic Support Keyworker you must be on the
Dynamic Support Register rated either Red, Amber or Blue.

You must be aged up to 25.

If you are over 18 your Dynamic Support Keyworker will support you as you move from children to adult services.

You must also have a GP in Hull, East Riding, North Lincolnshire,
North East Lincolnshire, York or North Yorkshire.

Dynamic Support Keyworkers will help you feel safe and happy.
Dynamic Support Keyworkers listen to you and hear you.
Dynamic Support Keyworkers help you plan what matters to you.
Dynamic Support Keyworkers help to make sure that you get the right help.
Dynamic Support Keyworkers work with you and others to plan for you to live well outside of hospital.
