Who are we?
We are the Community Forensic Team (CFT) in Humber Teaching NHS Foundation Trust. We are a multi-disciplinary team consisting of forensic specialists in psychiatry, nursing, social work, occupational therapy, learning disability and psychology.
We work with service users from Hull and East Riding. Service users will have involvement from other support services (Mental Health, Social Work) and will have committed an offence and/or pose a significant risk of harm to others.
We work with adult service users, carers and professionals to enable people to access specialist services and to be looked after closer to home. When required, we supported transitions for patients from CAMHS services.
Advice and Consultation:
Such referrals considered by the CFT may result in advice, such as signposting to the services, or a period of more formal consultation.
Advice and Consultation is available for:
- Staff members and teams from within Humber NHS Foundation Trust
- Providers supporting service users from Hull or East Riding in both in and out of Secure Services.
Intervention (In-reach for Secure Services):
Interventions may include:
Specialist intervention will aim to reduce risk whilst supporting the service user to reach their individual goals, alongside their care team. In
reach is provided for service users in in-patient secure services with an
estimated date of discharge within 6-12 months, dependent on need.
Care coordination is provided for service users discharged from secure settings who continue to require support from the CFT (not available for LDA pathway).
Care Co-ordination includes assessment and management of risk.
Specialist risk assessment will be considered when the consultation process has indicated that a forensic case formulation is not sufficient in understanding risk / need, and so further assessment work is required.
As a Humber Trust professional you may consider a referral to the service for advice and/or consultation if:
• You are concerned that a service user’s current mental health relates to a significant risk of harm to others, and it is likely that they are at risk of committing an offence.
• You have concerns regarding a criminal justice process; you might have questions about criminal justice options and potential pathways. You may be concerned about a service user receiving a custodial sentence and the implications on their mental health.
• You are working with someone who has a forensic history and would like advice about support, pathways, further assessment or discharge planning.
As a provider supporting a service user in a secure services you may consider a referral to the service if:
• Professionals or the service user require advice about future community support pathways and options.
• Professionals or the service user require advice about a repatriation to local secure in-patient services.
• A formal consultation may be required in the context of public protection processes such as a Parole Board hearing.
• In-reach is required to support a discharge to the community, if the service user is expected to be discharged within the 0–6-month criteria.

SPA will review your referral to see if it is suitable or not suitable for triage. If unsuitable, they may offer suggestions as to where to refer. If suitable, they will offer a consultation which will result in forwarding the referral to the following pathways:
At the Forensic Community Teams Referral meeting, further discussion will be held, and a decision is made detailing how the referral will be progressed.
Consultation:
A consultation will be offered by a CFT professional. Possible outcomes of consultation could be the following:
• Discharge with Recommendations (advice/signposting)
• Ongoing Consultations
• We will conduct consultation for service users who live out of area who are planning to settle in the Hull and East Riding area
Forensic Risk Assessment and/or Formulation
Intervention
• In-reach for Secure Services
• Joint interventions with other services
• Psychology, Nursing , Social Work and Occupational Therapy
Care Co ordination for MI/PD pathway only.
