Mental Health Inpatient Care Services Survey
Why have I been asked to complete this survey?
We are keen to learn about the experiences of those who have been cared for on our Mental Health Inpatient units, whether these are positive or negative.
We would encourage you to complete this survey as your responses will be used to help us identify things that are going well and things that can be improved.
When does this survey take place?
Individuals will usually be asked to complete this survey during their admission or occasionally following a discharge from inpatient services.
How do I access this survey?
The survey is available via MS Forms. You will be provided a link or a QR code to access the survey online by the team caring for you. If you would prefer to complete the survey on paper, please speak to a member of your clinical team who can arrange this for you.

We want you to feel confident being honest in your responses to this questionnaire - your answers will be anonymous and won’t affect any future care or treatment that you receive.
You will be asked which inpatient unit you are receiving care from to enable us to provide the feedback to the team looking after you.
What will happen to my feedback following completion of this survey?
We will regularly review responses provided through this questionnaire.
All feedback will be shared with the relevant team. Feedback which identities areas for improvement will be considered to ensure that we are providing the best care we can on our inpatient units.
