Next step in GP transition agreed

Humber Teaching NHS Foundation Trust is transitioning GP services at Market Weighton Practice, King Street Medical Centre in Cottingham, and Humber Primary Care in Bridlington to new providers due to changes in the national primary care model. Patients do not need to take any action.
Heads of Terms have been completed with the intention of transferring the practices to the following providers. This means the Trust and others have agreed on how to negotiate and recorded their intentions before a formal contract is finalised:
• Humber Primary Care to Drs Reddy and Nunn
• King Street Medical Centre to Greengates Medical Group
• Market Weighton Practice to The Ridings Medical Group
A formal letter will be sent to all patients explaining the transition, confirming the new provider, and detailing registration options.
The transition is being carefully managed, and a communications strategy will ensure patients and stakeholders remain informed.
This decision was made in collaboration with representatives from the Integrated Care Board and aligns with their strategy to support integrated neighbourhood care. It reflects a shared commitment to commissioning services that are sustainable, future-focused, and able to meet population health needs over the long term.
The shift of services from hospitals to community settings is also in line with the NHS long term plan and the government’s three strategic priorities for the NHS.
This process will be carefully managed to ensure continued care. The practice services will continue to be delivered throughout.
