Adult Mental Health Dialectical Behaviour Therapy Supports individuals to reduce the frequency of life-threatening behaviour, in particular self-harm and suicidal behaviour, and to develop a life that the individual feels motivated to remain alive for whatever that looks like for them. We often work with individuals who experience emotional dysregulation, when emotions feel extremely strong, are prompted very easily and quickly, and can lead to various difficulties for the person in their life. 77 & & 77a Beverley Road, Hull. Adult Mental Health PSYPHER (Psychosis Service for People in Hull and East Riding) An early intervention in Psychosis Service. Adult Mental Health The Homeless Mental Health Team • The Homeless Team – supports people who are street homeless or rough sleeping, living within a hostel, or on the edge of homelessness. The service aims to offer mental health support to individuals who have difficulties accessing mainstream mental health services.

• The Housing Support Team – the service aims to prevent people losing their tenancy as a result of mental health difficulties. The aim of Housing Support is to offer a mental health assessment to link people in with the appropriate agency for ongoing support.

Adult Mental Health Veterans Mental Health and Wellbeing Service Op COURAGE helps veterans, service leavers, reservists, and their families with mental health support in the Humber and North Yorkshire. Single point of contact across north of England.  Lead provider is Cumbia, Northumberland, Tyne and wear NHS Foundation Trust.
Prevention Recovery and Wellbeing Mental Health Recovery Wellbeing Coaches  Support the move from surviving to thriving by helping people feel in control of your health and wellbeing by addressing mental health, general health, wellbeing and inclusion needs. Based with community mental health teams.  Services delivered across Hull and east Riding  GP surgeries
Prevention Recovery and Wellbeing Health and Wellbeing Connector Service The Health and Wellbeing Advisor and Connector service is able to offer person centred support and link residents with local community groups and other vital services that offer support and advice to help them, improving self-care and tackling the social causes of ill health and wellbeing. Based within various community settings across East Riding including, health trainer shops, libraries, community centres and GP sites. The team also deliver outreach sessions in rural areas to ensure everybody can access the service.
Prevention Recovery and Wellbeing Fisherman’s Service Provides support and signposting for fishermen down the East Coast and in Hull and Grimsby. Communty outreach beased at the harbourside in Scarborough, Bridlington, Withernsea, Hull and Grimsby
Older People’s Mental Health Services Crisis and Intervention Team for Older People Inclusive of the Acute Community Service An alternative treatment option to inpatient admission when someone has an increased mental health need.  If the person needs inpatient admission, we work closely with the ward team and colleagues from other statutory and voluntary agencies to help them to go home as soon as possible. Older People’s Mental Health Services Hull Memory Assessment Clinic We offer an early memory assessment and treatment service through the Hull Memory Clinic. This is a specialist service for people of all ages, aiming to meet the needs of people who are concerned they may have a memory problem.  It is a multi-disciplinary team, qualified in diagnosing and treating memory problems, comprehensively assess and diagnose the nature of a person's memory difficulties. 
Following a diagnosis, a range of treatment interventions can be offered and can include medication if necessary. All of this is provided with additional input from a range of support workers and robust links with other agencies.

Mental Health Inpatient Units Mill View Court An acute treatment unit for working age adults who are experiencing mental health difficulties. Mental Health Inpatient Units Mill View Lodge A 9-bedded acute inpatient unit for men and women over the age of 65 and are presenting with a functional mental illness. Mental Health Inpatient Units Westlands Provides accommodation for 16 women aged from 18 to 65. The team includes nursing, medical and psychology staff who carry out a range of mental health assessments.  Wheeler Street, Hull, HU3 5QE
Forensics Forensic Inpatient Services The inpatient service provides secure mental health services for two levels of security (medium and low) each of which provides a range of physical, procedural, and relational security measures to ensure effective care and treatment whilst providing for the safety of patients (the ‘men’), the workforce and the general public. All services operate within the ‘community to community’ pathway. The Care Programme approach is used as a framework in which to plan and deliver care and treatment. Forensics Community Forensics The service supports those patients that originate from Hull and the East Riding of Yorkshire to improve timely transitions and reintegration of service users from secure care to the community.  The Community Forensic Teams work in collaboration with partners, service users and carers to provide in-reach and specialist community support to reduce admissions and referrals to secure inpatient care as well as reduce the length of stay in inpatient secure care.

NB Service also provided for North Lincolnshire and North East Lincolnshire Places.

Drug and Alcohol (adults) Case Management A structured support system with prescribed and psychosocial interventions, where a recovery plan is customised and reviewed regularly to support individuals on their recovery journey.
Drug and Alcohol (adults) Aftercare A 12-week one-on-one aftercare program aimed at equipping individuals with social and psychological skills for long-term recovery and reintegration into the community.
Drug and Alcohol (adults) Primary Care Addictions Service This service works with prescribing GPs to help manage patients experiencing substance misuse, expanding accessible treatment locations across the region.
Drug and Alcohol (adults) Optimise Focuses on helping individuals reduce or cease long-term prescribed medications, particularly analgesics and benzodiazepines, to mitigate health risks associated with prolonged use.
Drug and Alcohol (adults) Criminal Justice Team Integrated with local criminal justice services, this team ensures continuity of care for individuals engaged with the justice system, both in community and custodial settings.
Drug and Alcohol (adults) Community Rehabilitation Programme A 12-week abstinence-based day program offering tools and support for those reintegrating into the community post-recovery.
Drug and Alcohol (adults) Inclusion Health Team Reaches out to homeless individuals and those recently released from prison using a mobile unit ("Inclusion Health bus") that provides health education, overdose prevention, and needle exchange services.
Drug and Alcohol (adults) Changes Project (Service User and Family Support) Offers resources for service users, families, and carers, including SMART mutual aid groups and the Mentor Volunteer Programme to support both recovery and volunteer engagement.
Drug and Alcohol (adults) The Juice Bar (Image & Performance Enhancing Drugs) A confidential service targeting users of image-enhancing drugs (such as anabolic steroids), offering advice and information on associated health risks and addiction support.
Drug and Alcohol (adults) Parenting under Pressure (PuP) A therapeutic program to assist parents managing stress due to substance misuse and other challenges, supporting better family dynamics and parental wellbeing.
Drug and Alcohol (adults) Dual Diagnosis The dedicated Dual Diagnosis team lead and coordinate complex case management. They address the wide variety of conditions associated with people that present with co-morbid substance misuse and mental health issues.
Drug and Alcohol (young peoples service) Core Team This multidisciplinary team ensures effective case management and works closely with the Child & Adolescent Mental Health Service (CAMHS) for young people dealing with both substance misuse and mental health concerns.
Drug and Alcohol (young peoples service) Drug and Alcohol (CYP) Provides focused interventions for young people vulnerable to criminal or sexual exploitation.
Drug and Alcohol (young peoples service) Education and Liaison Engages with schools and colleges to educate young people on substance misuse risks, harm reduction, and resources for further support.
Community Services (adults) Whitby Community Hospital Whitby Community Hospital is managed by the Trust and provides inpatient, outpatient and community services to the town of Whitby and surrounding area. The Memorial Ward provides rehabilitation services and step-up/step-down from secondary care to maximise the patient's ability to live as independently as possible. The ward also provides palliative care and symptom mangement to those nearing end of life.

Forensics Forensics Single Point of Access (SPA) The Humber & North Yorkshire HNY SPA manages community and inpatient forensic referrals for the population of Humber, North & North East Lincolnshire, and York & North Yorkshire. Referrals for the forensic community teams are also processed through the SPA. The SPA’s role is to triage, coordinate and monitor all gatekeeping referrals for access to low and medium-secure beds and the forensic community teams.

Community Services (adults) Community Specialist Services - Tissue Viability CNSs The service provides specialist advice and care to patients with, or at risk of developing, wounds. This includes patients with wounds such as pressure ulcers (bed sores), leg ulcers, foot ulcers and unhealed surgical wounds as well as providing advice to patients who may be at risk of developing wounds, e.g. people who are immobile, diabetic or who have poor circulation. Community Services (adults) Community Specialist Services - Community Stroke Service - Including Early Supported Discharge (ESD) Community-based integrated stroke rehabilitation service for stroke patients. The service also supports early hospital discharge for stroke patients into community stroke rehabilitation bed and to their usual place of residence. Children's Services Youth Recovery and Wellbeing College (YRWC)  The Youth Recovery & Wellbeing College is a unique platform offering a 24/7 programme for those aged 11-18 living in Hull or ER and designed to engage and support the mental health of young people through a range of non-clinical and creative face-to-face sessions. It is led by young people through co-production and explores creative and empowering ways to improve wellbeing through digital and face-to-face sessions, workshops, courses, podcasts and team building activities. The Youth Recovery & Wellbeing College has a key ethos: we celebrate diversity and inclusion providing safe environments and platforms for young people to grow and gain new experiences.  The service does not have an address as they are community based.
The service currently delivers groups in various locations across Hull with an online offer for ER.
Children's Services Humber Youth Action Group (HYAG)  The HYAG is a youth group within HTFT for young people aged between 11-25 who live in Hull or the ER. The group meets for online sessions every 3 weeks.   This is a group that meets online so no physical base. 
CAMHS Hull and East Riding Long-Term Health Conditions Service (LTHC)  The Children’s Long-Term Health Conditions Team (LTHC) provides support to families and young people living in Hull and the East Riding of Yorkshire who are having difficulty psychologically adjusting to, adapting to, or managing their long-term health condition. CAMHS Hull Children’s Wellbeing Practitioners (CWPs)  Child Wellbeing Practitioners are part of the wider early intervention offer locally and are trained in delivering low intensity Cognitive Behavioural Therapy (CBT) to support young people up to the age of 18 to manage the difficulties they may be facing. This is offered in a range of settings including local health centres, youth centres, schools in young people’s homes or online. CAMHS My Mental Health Support Team’s (MyMHST)  MHST's provide early intervention mental health support for children and young people aged 5–18 in specific schools and colleges in Hull and the ER. The teams work across education and healthcare and provide consultation to support schools to identify and manage issues relating to mental health and wellbeing, as well as delivering high quality evidence-based interventions for children, young people and families who experience mild and moderate needs and can request specialist support or signposting as necessary.  The service does not have an address as they are community based.

CAMHS Contact Point – Hull and East Riding  Contact Point is a specialist service for children and young people up to the age of 18 that operates 24 hours a day, 7 days a week and is a team of experienced clinicians in mental health and learning disability who provide a single point of access for emotional and mental health referrals. The team will help to find the right service for a young person and family to ensure that they get the right support, at the right time and in the right place.  The service does not have an address as they are community based.
CAMHS Hull & East Riding Community Core CAMHS  The Core CAMHS service works with children and young people and their families, parents/carers as well as with other services such as schools, colleges and social care and has clinical care pathways in place to help with problems including depression and mood, eating difficulties, self-harming, severe behaviour problems and psychosis. A comprehensive range of services and interventions are available in a variety of community settings across Hull and the ER. A Key worker is allocated to remain with each family throughout their time with CAMHS. CAMHS CAMHS Eating Disorder Services  This team consists of health professionals that specialise in the assessment, intervention, and support of young people aged up to age 18, and their families, with a suspected or confirmed eating disorder. Neurodiversity Services Hull and ER Children’s Assessment Team Attention Deficit Hyperactivity Disorder (ADHD) /Autistic Spectrum Disorder (ASD)  The team completes Autism and ADHD assessments for children and young people who are below the age of 18 and have a GP in Hull or the East Riding of Yorkshire. The service generally assesses children for Autism from the age of 3 years old and children for ADHD from the age of 6 years old. 
Neurodiversity Services Hull and ER ADHD Interventions Team  This service provides post-diagnostic support and on-going management for children and young people with ADHD and their families.
Neurodiversity Services Children’s Sensory Processing Service  The Humber Sensory Processing Service is a specialist service providing support to children who have sensory processing difficulties. This service is for children with a Hull or East Riding GP. The Sensory Processing Service is a small team of sensory trained therapists. Neurodiversity Services Humber and North Yorkshire Dynamic Support Keyworker Service  Our Dynamic Support Keyworkers work with autistic people and people with a learning disability aged up to 25 years who are on the Dynamic Support Register rated RED, AMBER or BLUE (inpatient).
The Dynamic Support Register is to identify people at risk or may become at risk of admission to a mental health hospital without support. Dynamic Support Keyworkers make sure that people and their families/carers get the right support at the right time. They aim to lower admissions to mental health hospitals and support people leaving hospital.

Neurodiversity Services Autism Mental Health Support Service The Autism Mental Health Support Service (AMHSS) work with autistic service users, their families, carers and mental health colleagues across Humber Teaching Foundation Trust, to offer support during a deterioration in their mental health. Children's Therapy Service Children’s Occupational Therapy The Occupational Therapists help children and young people to reach their full potential and develop life skills so they can join in and succeed in activities that are important to them, helping them take part in everyday activities (occupations) that they may find difficult because of their illness, injury or disability. These include self-care (getting washed/dressed, using the toilet or eating a meal), being productive (access to learning, handwriting/using scissors or learning through play at nursery) and leisure (play or hobbies). The service has 3 bases although will work with the child young person and family in the most appropriate setting for them such as the home, school/college/early years setting and community health settings including out patient departments.

Learning Disability Services Community Team for Learning Disabilities (CTLD) The Community Team for Learning Disabilities (CTLD) provides specialist health support for adults with learning disabilities whose needs cannot be met through mainstream services. Although the home is a specialist service within Humber’s Learning Disability Service, Granville Court is not a hospital, and the ethos is very much that it is a person’s home for life.

Learning Disability Services Townend Court Inpatient Unit Townend Court is an assessment and treatment unit (ATU) for people with a learning disability with additional mental health needs, whose needs cannot be managed within the community. At Townend Court, the person’s mental health can be fully assessed in a safe place, by a specialist team of health professionals.
They offer specialist support for people with a learning disability, autism and mental health diagnoses and / or behaviour of concern. They offer a safe environment for care and treatment for people who have become unwell and need to come into hospital for help.
People who are admitted to the unit, have been detained under the Mental Health Act (2007) or as an informal patient.
People are admitted into Townend Court when assessment and treatment cannot safely be provided in the community.

In Hull the Infant Feeding Service is delivered by maternity, integrated public health nursing service (IPHNS) and Family Hubs. The Infant Feeding Service is here to provide you with information and support on feeding and caring for your baby throughout pregnancy, birth and throughout your child’s first formative years.

Universal Children's Services East Riding Health Visiting and School Nursing  The Integrated Specialist Public Health Nursing Service (ISPHNS) 0-19 delivers the Healthy Child Programme (HCP) ‘Pregnancy and the first five years of life’ and ‘From 5-19 years old’. Our service provides specialist health support, which starts at the point of pregnancy and continues throughout the child’s life until they reach the age of 19 or up to the age of 25 for people with special educational needs and/or disabilities. 
The service is delivered by six professional teams across the East Riding of Yorkshire.
