National Social Prescribing Day 2020

The East Riding Social Prescribing service is celebrating! From Monday 9th to Friday 13th March the team will be traveling throughout the East Riding to celebrate National Social Prescribing day.

Having seen over 8000 service users in the past 2 years, the service is looking to showcase the work it delivers week-in, week-out for East Riding residents.

Then the social prescribing service is ready to help!

With local partners joining us each day of the week; ranging from The Carers Service, ERYC Sport and Play service, Citizens Advise and Community Mental health teams. Come along and ask how we can support and help you.

The team are based across the East Riding in all GP practices and can help you with the following:

  • Substance misuse, including Alcohol.
  • Stress and anxiety
  • Physical activity
  • Isolation and loneliness
  • Bereavement
  • Welfare
  • Housing
  • Healthier lifestyle

The team offers up to four one-to-one confidential sessions to all who sign up. The Community Link worker is available to listen, support and provide guidance to help you overcome any barriers, stressors and issues in your life.

Natalie Belt, Service Manager at Health Trainers, Humber Teaching NHS Foundation Trust, said:

“Day to day life can be as stressful as ever and sometimes you may feel lost or unsure where to turn to for help. The Community Link team are there to help and support you during this time and help you get back on track.

Our teams are experienced in helping residents with a wide range of issues, including their emotional wellbeing, lifestyle choices, welfare and housing problems. They provide time and a listening ear that guides and supports you with your problems, however big or small they may be.
