Scarborough and Ryedale Integrated Prevention, Community Care and Support Service
Providing Community care services for the population of Scarborough and Ryedale on 1 May 2018.
The Team
Our Integrated Prevention, Community Care and Support Services include:
- Cardiac CNSs and Cardiac Rehabilitation Service (patient classes)
- Community Stroke Service
- Community therapies: physiotherapists, occupational therapists, and generic support workers (8am—10pm)
- Continence Service and Continence Product Service
- Customer Access Service (CAS)
- Diabetes CNSs and Diabetes Education Service
- Dietetic Service
- District nurses: nurses and healthcare assistants (8am—10pm)
- Frailty - Holistic, multi-agency community pathway, including Pharmacy support and Elderly Medicine
- Heart Failure CNSs
- Home Oxygen Delivery Service
- Hospital Discharge Service (HDS)
- Inpatient Ward – Fitzwilliam Ward, Malton Hospital
- Muscular Skeletal Outpatient Service (MSK)
- Rapid Response: Multi-Disciplinary Community Team support (8am—8pm) and including Diversionary Pathway with Yorkshire Ambulance Service
- Respiratory CNS Team and Pulmonary Rehab Service (patient classes)
- Speech and Language Therapy
- Tissue Viability CNSs
- Urgent & Unplanned Nursing Service (Scarborough)

Opening Times
Monday to Sunday, 8am – 6pm. The team works in conjunction with the Yorkshire doctors to create a 24/7 service.
