Providing Community care services for the population of Scarborough and Ryedale on 1 May 2018.

The Team

Our Integrated Prevention, Community Care and Support Services include:

  • District nurses – nurses / healthcare assistants
  • Community therapies – physiotherapy / occupational therapy / generic support workers
  • Respiratory CNS Team, Pulmonary Rehabilitation and Home Oxygen Services
  • Cardiac clinical nurse specialists and Cardiac Rehabilitation Service
  • Heart Failure clinical nurse specialists
  • Diabetes clinical nurse specialists and Diabetes Education Service
  • Continence Service and Continence Product Service
  • Tissue Viability clinical nurse specialist
  • Dietetic Service
  • Speech and Language Therapy
  • Muscular Skeletal Out Patient Service (MSK)
  • Community Stroke Service

Opening Times

Monday to Sunday, 8am – 6pm. The team works in conjunction with the Yorkshire doctors to create a 24/7 service.
