Veterans outreach service

Soldier wearing a poppy

Mental health triage and assessment for military veterans registered with GPs across Yorkshire and the Humber.

The Veterans Outreach Service is a new mental health service to support veterans and their families. The service developed from a recognition that military veterans may find it difficult to access traditional NHS services following time spent in a military environment and acknowledges that a stigma still surrounds mental health issues and aims to reduce this by providing more accessible pathways into services.

The service provides a first point of contact for people aged 18 and over and is available to anyone who has completed one day’s service or more with the British Military, is a Reservist not currently deployed or is a family member of military personnel.

The outreach team offers mental health assessment by a specialist veterans mental health outreach therapist who is sensitive to military culture and has good local knowledge.  The service aims to provide thorough assessment to refer veterans onto the most appropriate service to meet their needs. Service leaflets can be downloaded on the link on the right hand side for more information.

The veterans outreach service has an open referral system and people can refer themselves by contacting the service directly on 01482 617594.  Referrals can also be taken from any statutory or non-statutory organisation.  Referral forms can be downloaded by clicking on the attachment to the right of the screen.

Humber Foundation NHS Trust is the host of the service. However, the service is run in partnership with Leeds and York Partnership Foundation NHS Trust and Sheffield Health and Social Care Foundation NHS Trust.  Please see 'Related sites' on the right for further information on these organisations.
