Our Integrated Prevention, Community Care and Support Services include:

Cardiac CNSs and Cardiac Rehabilitation Service (patient classes)
Community Stroke Service
Community therapies: physiotherapists, occupational therapists, and generic support workers (8am—10pm)
Continence Service and Continence Product Service
Single Point of Contact (SPOC)
Diabetes CNSs and Diabetes Education Service
Dietetic Service
District nurses: nurses and healthcare assistants (8am—10pm)
Frailty - Holistic, multi-agency community pathway, including Pharmacy support and Elderly Medicine

Muscular Skeletal Outpatient Service (MSK)
Rapid Response: Multi-Disciplinary Community Team support (8am—8pm) and including Diversionary Pathway with Yorkshire Ambulance Service
